Prostate cancer patient

Dad at 70 years of age has been diagnosed with prostate cancer. I need to write down the timeline in order to remember and track progress.

May 2nd, 2023 - blood test

May 16th, 2023 - doctor appointment (cholesterol went up from 7 to 7.4mg/dl)

July 2nd, 2023 - blood test

July 20th, 2023 - PSA 5.79 vs 5.2 in April blood test

Nov 17th, 2023 - MRI

Dec 4th, 2023 - MRI report (Dr had suspicion from report, advised to do prostate biopsy)

Jan 16th, 2024 - Biopsy at private hospital (lots of bleeding at first)

Feb 13th, 2024 - IHC report (< 5% cancer tumor size, Gleason score 3+4=7 grade)

Feb 19th, 2024 - Urologist and superior advised active surveillance and have PSA test in 3 months and monitor.

Apr 30th, 2024 - blood test

May 14th, 2024 - urologist appointment (PSA 6.76 vs 6.59 in Jan 2024) Dr said very slow increase unless PSA above 10.

Jul 30th, 2024 - PSA blood test

Aug 13th, 2024 - PSA results

Most recent PSA result came back with 7.47, which means PSA level is rising. At this pace, my dad requested for another biopsy himself. (a transrectal biopsy)

Biopsy result came back in September 2024 with the following:

 

(D) Left prostate for HPE (APEX)

Tumor present, 1 out of 2 cores, tumor volume 10%

Histological diagnosis: Adenocarcinoma of prostate, Gleason score 6 (3+3), Grade group 1

 

(E) Left prostate for HPE (MID)

Tumor present, 3 out of 3 cores, tumor volume 10%, 10%, 90%

Histological diagnosis: Adenocarcinoma of prostate, Gleason score 7 (4+3), Grade group 3

Percentage Gleason 4: 75%

 

It is considered unfavorable, intermediate risk of prostate cancer. A Gleason score of 4+3 is a moderately aggressive form of prostate cancer.

The doctor suggested HIFU (High-Intensity Focused Ultrasound) as a treatment, if PET scan and MRI both show no metastasis.

So, my dad will be taking a PET scan in 2 weeks and MRI in order to be qualified for HIFU, which is a relatively new treatment. I really hope that he will be able to undergo this surgery, because I know he wants to and the surgery will kill all of the cancer to give him many more years of life.

He is currently in the hospital waiting to be treated either with HIFU or Nanoknife. Prior to this, He did PET scan (public hospital) and MRI (private hospital), both qualified him for HIFU. 

 

MRI Findings:

Prostate size 2.7 x 4.7 x 2.4 cm (AP x W x CC), approximately 15.84cc.

Lesion #1:

Location: Left peripheral zone at apex level.

Category: 4

 

Lesion #2:

Location: Left transitional zone posterior at mid gland level.

Category: 2

 

Impression:

Left prostatic carcinoma.

No extracapsular extension.

No nodal or distance metastasis.

 

He didn't receive HIFU treatment free of charge as initially told, but now has to pay out of pocket for it.
